Workload:
Workload of student for successfully completing the course. Each ECTS credit represents 30 working hours. These are the combined effort of face-to-face time, post-processing the subject of the lecture, exercises and preparation for the exam.

The total workload is distributed on the semester (01.04.-30.09. during the summer term, 01.10.-31.03. during the winter term).
90 class hours (= 67.5 clock hours) over a 15-week period.
The total student study time is 150 hours (equivalent to 5 ECTS credits).
There are therefore 82.5 hours available for class preparation and follow-up work and exam preparation.

Learning outcomes:
After successfully completing this module, students will:
_ be qualified to analyze, organize and control nursing care and service processes in different healthcare institutions/settings according to individual needs and on a case-by-case basis.
_ be able to assess functional structural requirements and distinguish between the different dimensions and fields of action in case management.

Module content:
Based on the recommendations of the GKV-Spitzenverband according to § 7 a para. 3 sentence 3 SGB XI on the number and qualification of nursing consultants from August 29th, 2008:
 
Theoretical and practical principles of case management:
- Definitions and functions of case management
- Case management concepts
- Collaboration and networking in interdisciplinary cooperation
- Resource analysis and resource assurance
- Concepts for demand assessment and supply management
- Field of action-specific theories and applications
 
 
Advanced study in specific fields of work:
- Intervention logics to implement existing legislation
- Nursing care case management
- Case management in outpatient, long-term inpatient care settings
- Creating care plans taking into account recognized classification systems (e.g. ICF)
- Care plans as a means of case management
- Knowledge of social law procedures (administrative act, opposition, complaint)
_        Principles of social-legal action (§§ 12ff SGB X)
_        Advisory assistance according to § 37 para. 3 SGB XI
_        Concepts of integrative work with relatives
 
_        Decision-making processes _ Change processes and their dynamics
